FinalForms:

FinalForms is an online application that will enable parents and students to complete and sign athletic forms online and is available for all athletes in grades 7-12 at: http://austintown-oh.finalforms.com/.  Instructions can be found here:  FinalForms Parent Playbook

Each athlete will have TWO accounts.  1. Parent Account and 2. Student Account.  When forms are being signed each year, they must be completed from the Parent Account first, followed by the Student Account.

Initial registration will take around 10 minutes.  Information is stored from season-to-season, year-to-year and also shared within families eliminating the need to enter the same info more than once.  Your only annual requirement will be logging in to verify the existing information.  Registration Instructions.
